Instrument Assisted Soft Tissue Mobilization (IASTM) & Gua Sha

At the Robert McDonald Clinic, we use specialized tools to “scan” and treat the soft tissues of the body. Whether using traditional medicinal stone or modern medical-grade steel, these techniques are designed to break down scar tissue and “reboot” the healing process in chronic injuries.

The Science: Mechanotransduction & Microcirculation

When a tool is glided over the skin with specific pressure, it creates a “micro-vascular” response that manual massage cannot replicate.

  • Fibroblast Activation: The pressure from the tool creates a microscopic “insult” to the tissue, which signals the body to send fibroblasts to the area. These cells are responsible for laying down new, healthy collagen fibers.
  • Upregulating Heme Oxygenase-1 (HO-1): Research has shown that Gua Sha significantly increases the production of HO-1, an enzyme with powerful antioxidant and cytoprotective (cell-protecting) properties.
  • Resolving Myofascial Adhesions: Over time, stress and injury cause fascia (the body’s “cling wrap”) to become sticky and thick. IASTM “combs” through these fibers, restoring the “glide” between muscle layers.

The Evolution of the Tools

We utilize three distinct styles of instrument therapy depending on the clinical goal:

Traditional Gua Sha

  • The Tool: Typically made of buffalo horn, jade, or smooth resin.
  • The Focus: Used primarily for systemic health, such as “venting” a fever, treating respiratory issues, or clearing “heat” from the body.
  • The Result: It often produces Sha (petechiae), which indicates where the blood flow was stagnant.

IASTM (Modern Sports Medicine)

  • The Tool: High-grade, beveled stainless steel instruments (similar to Graston or Gaston techniques).
  • The Focus: Deeply anatomical. We use the resonance of the steel to “feel” vibrations in the tissue that indicate fibrotic scar tissue or “gristle” in the muscle.
  • The Result: Excellent for Plantar Fasciitis, Achilles Tendinitis, and Carpal Tunnel Syndrome.

Bian Shi (Stone Medicine)

  • The Tool: High-grade, beveled stainless steel instruments (similar to Graston or Gaston techniques).
  • The Focus: Deeply anatomical. We use the resonance of the steel to “feel” vibrations in the tissue that indicate fibrotic scar tissue or “gristle” in the muscle.
  • The Result: Excellent for Plantar Fasciitis, Achilles Tendinitis, and Carpal Tunnel Syndrome.

What to Expect

The Sensation: You will feel a firm “scraping” or “stretching” sensation. It is rarely painful; most patients describe it as a “satisfied” feeling of a deep itch finally being scratched.

  • The Appearance: Like cupping, Gua Sha can leave temporary red marks (Sha). These are not bruises, but “transient therapeutic petechiae” that show the body is successfully moving stagnant metabolic waste. They typically fade within 2 to 4 days.
  • The “Grit”: You may feel a “sand-like” texture under the tool as it passes over an old injury. This is the tool detecting the uneven texture of scar tissue, which we then work to smooth out.
